Welcome to the website of the North Haven Middle School Health Office. Here you will find a general description of some of the health office policies, as well as the most commonly requested forms.

All students wishing to try out for a sport at North Haven Middle or High School must be registered online by their parent or guardian via FamilyID.com.  This procedure is required for every sport and must be renewed every school year prior to tryouts.  Sports physicals must be done annually, and uploaded into the registration (preferred method), or handed into the school nurse prior to tryouts.

When Are School Physicals Required?

Physicals are certain immunizations are required for entry into Preschool, Kindergarten, and grades 7 and 11. The physical must be completed on the blue "State of Connecticut Health Assessment Record", which can be found in the Forms section. Students newly enrolled must also meet the appropriate requirements for the grade they are entering.



Health screenings are done by the school nurse and/or trained volunteers, which may include school staff members, according to the following schedule:

  • Vision (distance only)- Kindergarten and grades 1,3,4 and 5
  • Hearing- Kindergarten and grades 1,3,4 and 5
  • Postural (scoliosis, or spinal curvature)- Females grades 5 and 7 / Males grades 8 or 9
